CERTIFICATION OF COMPETITIVE ELIGIBILITY PROCESS - SUMMARY OF FORMS

Required NCAA paper work is administered by the compliance office at the beginning of each athletic year or when first reporting for a sport. The following is a list of the forms distributed including a summary of due dates and content. VT Student-Athlete Historical Report:Important information including high school(s) attended and history since graduation. This form is to be completed one time by a student-athlete, as a freshman or when first reporting for a sport and is kept in the student-athlete's file. Summary of NCAA Regulations:Provided to all student-athletes at the beginning of each athletic year or when first reporting for a sport. This packet includes important information provided in two parts the first for all student-athletes and the second for new student-athletes only.
  • Part I:Ethical conduct, amateurism, financial aid, employment earning, academic standards, continuing eligibility and other eligibility regulations. Part II:Recruitment, contacts, publicity, letter-of-intent signing, source of funds, tryouts, sports camps, visits, transportation and entertainment, and precollege or postgraduate expenses.
Virginia Tech Authorization for Release of Information (HIPAA):Important information granting the release to specified individuals prior to the first day of practice as a member of an intercollegiate team at Virginia Tech. NCAA Authorization/Consent for Disclosure of Protected Health Information (HIPAA):Important information granting the disclosure to specified individuals prior to the first day of practice as a member of an intercollegiate team at Virginia Tech. NCAA Student-Athlete Statement:Must be completed by student-athletes and on file prior to the first day of practice as a member of an intercollegiate team at Virginia Tech.
  • Part I:The student-athlete affirms that they meet the NCAA regulations regarding eligibility, recruitment, financial aid, amateur status, and involvement in organized gambling and are aware of the NCAA drug-testing program; Part II:The student-athlete signs the Buckley Amendment Consent Form which certifies that they agree to disclose educational records. Part III:For incoming freshmen only. The student-athlete affirms the validation of the submitted ACT or SAT score. Part IV:The student-athlete affirms that they have not tested positive for banned substances by a non-NCAA athletics organization and agree to drug testing by the NCAA if they have.
Virginia Tech Drug-Testing Consent:Must be completed by student-athletes and on file prior to the first day of practice as a member of an intercollegiate team at Virginia Tech. Student-athletes sign to confirm their awareness of the Virginia Tech drug-testing program and agree to Virginia Tech drug testing. NCAA Drug-Testing Consent:Must be completed by student-athletes and on file prior to the first day of practice as a member of an intercollegiate team at Virginia Tech. Student-athletes sign to confirm their awareness of the NCAA drug-testing program and agree to NCAA drug testing. NCAA Banned-Drug Classes:Student-athletes must review this document in conjunction with the NCAA Drug-Testing Consent Form prior to the first practice date. General Amateurism and Eligibility Form for International and Select Student-Athletes:For new international student-athletes only. Must be completed and on file prior to the first day of practice.
